How does referral work?

Firstly, we will ask a few questions to find out some details about the person who is looking to join us. We will make sure we are a good fit for them and have all the facilities they need to feel safe and comfortable. Furthermore, we will then provide you with details of the service we can provide and the fees, we like to make sure we are up front and transparent about what we provide and what the costs are. Towerview Care have two fee rates based on the person’s needs.  The fees do not include 1:1 hours which are charged at one rate.

Once we have the information we need and you are happy to proceed we will arrange for our Matron to visit you at home and carry out a full assessment of the persons needs and wants. 

Matron’s visit

Don’t worry our matron is coming out to meet the person who wants to join us, to make sure that we match all of their needs. The visit takes between 2 to 3 hours, we know that seems like a long time but this enables us to make sure that we know everything about the person, their family, friends and support network. This will make sure that we are aware of all their needs and have everything they need to move to their new home.

Once our Matron has been to visit you, she will then meet and discuss her findings with the Registered Manager and clinical team. Together they will determine whether Towerview Care is the right place for you or your loved one to live.

When you are offered a place we will make sure that the transition is as easy as possible. This will include visits to be introduced to everyone, personalising your room. Also practical arrangements such as a change in GP, updating of care plans, establishing communications and financial support and moving personal items.

Everyone is different and sometimes we need to do things differently, so we can offer immediate /urgent placements if circumstances require it. On average the placement starts within 1 week of the Matrons assessment.   Whilst it is not ideal it can be necessary in some cases. At all stages you or your loved one, family and the support network are fully involved and kept up to date.

Once we have the information we need and you are happy to proceed we will arrange for our Matron to visit  the person you are looking to pace with us at their current location.  Matron will carry out a full assessment of the persons needs and wants.  For Complex cases Matron may bring with her our Psychologist and or Occupational Therapist.

Referrals can be responded to on the same day or no later that 48 hours.

The assessment

The assessment visit takes between 2 to 3 hours,  this enables the team to make sure that we know everything about the person, their family, friends and support network. This will make sure that we are aware of all their needs and have everything they need to move to Towerview Care.

Once our Matron has been to visit, she will then meet and discuss her findings with the Registered Manager and clinical team,  together they will determine whether Towerview Care is the right placement.

Matron will then provide with the details of her assessment, fees and when the placement will be available.  This can be same day for urgent referrals or longer dependent on the persons needs and written confirmation of funding.

Q&A

Can I be helped to access funding for the placement?

Yes, we can support you to contact your local Authority or NHS (CCG) funders.  The process can take time and we will not be able to agree a move in date until its agreed. In addition, a financial assessment will determine who funds and at what percentage. There may be an expectation for a contribution to be made by the individual or their estate. This is covered in the financial assessment.

You may be funding the placement yourselves which is detailed in a contract between yourselves and Towerview Care. 

Can I visit anytime?

Yes, we have open visiting times.  We would only ask visitors to leave if the person they were visiting was becoming distressed or requested them to leave.

Moving In

Transition can be a challenging time for the person and  their family so it is imperative that we are all sensitive to this.

Care plans, risk assessments, medication and other support will be in place at the time of arriving at Towerview Care. Personalising their room will be undertaken with the person either before or within the first two weeks of moving in.

A review will be held after two weeks to see how the person is settling in and their needs are being met.
